Could anyone advise re question 3 that states 'attach a draft copy of the order you are applying for'

Is there another Form to download for this or is it just a written statement i do to the best of my ability/knowledge?

There is a compendium of standard orders - a series of adaptable templates to cover just about every order in the family courts. You can find it online.
Ideally you should get a solicitor to draft the order but if you can find a template that covers what you are asking for you may be ok. NB attaching a draft (ideally as a word doc) saves time and aggravation for the court; they aren't really geared up to draft orders these days. However if you are not certain you have the right template I would just write in simple plain language exactly what you are asking the court to order. As you are LIP the Court will prepare the order.
