Do I include MIAM with Form E?

I've finally finished form E which needs to be in this week. Im just wondering whether to include my MIAM certificate at the back of it. I'm the respondent and wanted to sort things through mediation but stbx wouldn't. I believe he has MIAM certificate from different mediator in order to start the court proceedings.

Are you in the court process or is this a voluntary exchange??

If you are in the court process the sealed form a (with mediation sign off) will be included in the bundle.

If voluntary, the sign off would signal that you are in a position to apply to court. NB it is valid for 4 months. It really isn't necessary to include it though.
