Help with Divorce Form and Financial form D81 and Form A

Hello, I am assiting a friend who's wife has filed for divorce and my friend will not be contetsing this. The wife has agreed that she will not be making a claim for finances as there are no marital assets, no children and it was a short marriage of 4 years, only lived with each other for 3 years. As my friend is now wanting to make an application for a Clean Break Consent Order, does he need to select NO when it asks in the divorce application 'Do you intend to ask the court to delay the divorce until it is satisfied with your financial situation?' When you select YES, it asks whether you will complete Form B.

He intends on filing a D81 and Form A - is this incorrect?

Also - he does not know where his wife is living, she instructed Solicitors for the divorce but not for the financial matters and has said to email her, what does my friend put on the form as her address?

Any guidance would be much appreciated
