If an exhibited document consists of eg 5 pages, is it acceptable to just number the 1st page or do each separate pages needed to be numbered if only 2 of the pages are relevant

You really need to number all the pages of all documents from 1 - end in a bundle of evidence. Think about the judge who will need to be able to navigate the statement and supporting evidence bundle. Ideally you should provide an index of exhibits and reference them by exhibit number and page in the statement.
E.g. Exhibit 3 (title of document) pages 7 and 8 of evidence bundle.

Is it just the updated replacement documents that need to be in the bundle for final hearing. eg updated ES1 & ES2 or do the out of date ones also need to be included

You put in the latest ES1 and 2.
If it is for final hearing you can put in open offers but not those made without prejudice.
Supporting documents from form E are not included routinely. Any that may be relevant need to be specifically included in an additional evidence section.

Practice direction 27a governs bundles.
