Consent order application fiddly bits

I am trying to fill out form D81 Statement of Information for a Consent Order and would appreciate help on calculating net income.

For myself I am using the following:-

Take home pay - less pension payments - less Child maintenence = Net Income

For my Ex wife we are calculating as:-

Take home pay + Child maintenence + Child Benefit + child tax credit = Net Income.

As long as you make clear on the form that that is how you have calculated it, that would be fine.

Thanks for explaining that and I will complete the form and send to PRFD.

I think I need to send a Form A 'Notice of an application for Ancilliary relief' marked for dismissal purposes with the D81. Do I have to tick any of the boxes or just write for Dismissal on the top left hand corner? do I need to send the £210 fee? Hopefully not as this seems steep just for dismissal.

How many signed originals & copies of the consent order does the PRFD require? Do they also need an unsigned copy?
