What do I send with my petition

Just doing my last minute checks before I send my Petition (D8) to the court, and I find in the wikivorce divorce guide this:

The process is commenced by the filing at court of,

- the petition for divorce

- the statement of arrangements for the children (a formal questionnaire)

- the original marriage certificate*

- a certificate as to whether or not the parties have attempted reconciliation

- a cheque for the court fee which is £300


Does this "reconciliation certificate" have a number please and can I download it from HMCS website? It has somewhat thrown me as I have seen no reference to it elsewhere.

UM you only need one if you are paying a solicitor, not if your LIP.
