Identify signature D80A Affidavit

Hello,

I am having a problem with the third page of the affidavit (80A) where you need to identify the signatures of the respondent.

The respondent has only signed the Acknowledgment of Service.

I have not received any documents marked "B" or "C" (statement of arrangement, though I did file one). Do I leave these spaces blank?

Thank you for any advice.

no you just cross through the paragraphs which are not relevant so if your ex has not signed the statement of arrangements at all, you cross out that paragraph.
PLEASE everyone remember that when you identify your ex's signature if he/she signs as J. Smith that's the words you use in the identification paragraph and if he/she actually signs John Peregrine Smith, that's what you put in.
