I have completed my divorce petition and am currently trying to complete the statement of arrangements for our children.

My xtb has recently moved and has not yet informed me of her new address (dont you just love their silly little games ;) ).

I therefore do not know the following information in the statement of arrangement form - where the children live, the size of the house, how she is paying for it, etc.

The form itself seems to me to be backward (or unintentionally bias) as it looks like the petitioner is the person who is the pwc, whereas in my case im the petitioner and the nrp (although time is split nearly 50:50 - she wanted the extra night)

With all of this info missing how do I go about completing it without it getting thrown our by the courts?

Well, you really need that information. When you exchange the children can you give her aletter requesting it and tell her why.

Also, do you have the marriage certificate? You need that to file the petition.

If she will not give the new address, complete it on the basis of the old address with a note explaining thqt she has recently moved, that you have requested the details and these have not been provided. Important. Don't make things up. Give her time to respond.

Originally we agreed on 2 years separation, but to be honest I find that just a complete mockery of marriage.

I have had to apply for a marriage certificate and have that now. She is not very impressed when I say that I am getting a divorce and it usually ends in arguements saying that she will contest it (why I have no idea) so I told that I have started the process and left it at that. She previously asked me to use her mum and dad's house as a forwarding address even though I know that is not where she lives.
