Pension sharing order - Is part of the wording in top left corner crossed out?

Can anyone tell me if part of the wording is crossed off on your order, top left corner?

My Pension Management Firm have refused the pension sharing Order because the words on the top left saying "paragraph 15 of schedule 5 to the civil partnership act 2004" have not been crossed out.

Nothing on the form says it has to be done. They've got a copy of the Decree Absolute which show's its was a marriage.

Any help appreciated.
